Texans who are registered with the vaccine registry, ImmTrac2, no longer must search high and low to find vaccine records. This is very convenient when you need to have your vaccine history for school, a job, or travel. You can find your children’s complete vaccine history in one place when it comes time to enroll them in daycare, school, sports or extra-curricular activities.
Enrollment in the Vaccine Registry
You are not automatically enrolled in ImmTrac2. It is offered to all residents of Texas at no cost. The vaccine registry is confidential and secure. When you are enrolled, no matter where you get your vaccines, they are recorded in the database. This means you can have records from multiple sources consolidated in one place. This comes in handy particularly when needing to gather vaccine information for school enrollment for the kids.
Your written consent is required before you can enroll or have your children enrolled. Only authorized individuals may have access to your information. Health care providers, schools and public health departments have the authorization to retrieve your records.
Requesting an Immunization Record
You can request records for yourself and your children through the vaccine registry. Once you are enrolled in ImmmTrac2, all you need to do is complete a form and send it to the DSHS Immunization Unit. The form is called the Authorization to Release ImmTrac2 History Form. Adults 18 years old and up must also sign an ImmTrac2 Adult Consent Form (EF11-13366)

From Childhood to Adulthood
Immunization records for children are kept by the vaccine registry from childhood until your 26th birthday. When a child who has been registered turns 18, he or she has until their 26th birthday to sign the ImmTrac2 Adult Consent Form (EF11-13366.) If the form is not signed by then, the immunization records will be deleted from the vaccine registry.
Removal from the Vaccine Registry
If at any time you choose to remove yourself, your child or your family from the vaccine registry you may do so by completing the ImmTrac2 Withdrawal Form. This form can be downloaded, or you can request one from the Immunization Branch by calling (800) 252-9152. After completing the form, you can mail it or fax it to the Texas Department of State Health Services using the information above.
